If you have used this online form before, enter your username, which is your Clinic ID # and your password. If you forgot your old password, click on the ‘No” button and enter your new password and click the ‘Continue’ button.

You will come to a page with a drop down list of all the faculty members you have been requested to evaluate. Select the faculty member to evaluate and click in the ‘Continue’ button and fill out the evaluation. Once you submit all the evaluations, your list will be empty and you can close the browser.

If there is a faculty member that you have not had contact with or you feel there is an error in the list of faculty to evaluate, contact Ms. Wang.
