The Graduate Program Application is available online and must be submitted that way.  Please follow the instructions carefully and note that you you may be called for an interview.

The application fee is $75 for the Ph.D. or M.S. program.  Combined program OD/MS or OD/Ph.D. applicants, who have previoulsy paid an OD application fee, should not pay any application fee.

If you are an M.S. or Ph.D. applicant and are sending in a fee, you may either use the online application form containing a secure credit card procedure, or, if you prefer, pay by check.  The check must have a U.S. bank on it.  U.S. travelers checks are ok.  If you pay by check, make it payable to:
